Patient Rights and Consumer Information

This practice is committed to transparency, client rights, and compliance with Texas law. The information below explains how clients may access their health care records, contact the appropriate licensing authority, and file a consumer complaint.

Requesting Your Health Care Records

Clients have the right to request access to their health care records in accordance with applicable state and federal law.

To request your records:

  • Submit a written request to this practice.

  • Requests may be made by email, secure client portal, or written letter.

  • Requests must include sufficient information to verify your identity.

  • Records will be provided within the timeframe required by law.

  • Reasonable fees may apply as permitted by Texas law.

Filing a Consumer Complaint

Clients may also file a consumer complaint with the Texas Office of the Attorney General regarding consumer protection concerns.
